Certification Procedures:

1. Send completed Level 1 Medication Aide Request for Examinations form and a completed Pre-Class Roster to the Missouri Assisted Living Association. Be sure to check the EDL (Employee Disqualification List) AND call 573-526-3871 for previous certifications. The EDL is now accessible via the DHSS website and DHSS Interactive Voice Response System. For more information about the EDL, CLICK HERE. If you add a student to the class after you have requested exams you MUST send that information to the MALA office.
 
2. You will receive the requested number of examination booklets, answer sheets and one score sheet. (The score sheet may be copied.)
 
3. Please return the following within 10 days after testing (if there is a change in test dates – please indicate it on your Final Class Roster.)
  • A Final Class Roster with attendance dates, total hours, and signatures.
  • A graded answer sheet and completed Score Sheet and Competency Sheet for each student (all blanks must be filled in and signed).
  • Answer grid and all examination booklets.

Level 1 Medication Aide: (Certificate, ID, and pin)

Failure to return tests and answer grid; or sending in incomplete Rosters, Score Sheets and/or Competencies; or failure to pay correct amount due will cause delay in processing certifications and/or requests.

Obtaining a Copy of Certifications
 
To obtain a copy of your Level 1 Med Aide certification certificate and/or your Insulin Administration certification card you must complete and submit our Duplicate L1MA/Insulin Certificate Form along with required payment and supporting documentation.
